What to Bring
Please bring the following to the Program Session:
- Your completed New Patient Questionnaire
You may download the New Patient Questionnaire
in Adobe Acrobat format. You may print a copy to complete offline, or you may complete the form within Adobe Acrobat. Please note that if you use Adobe Acrobat you will not be able to save your work. - Any prescription medications, vitamins and supplements that you are currently taking.
- Copies of any prior lab work and medical history relevant to your health and hormonal status.
- A laptop computer.
- A printed copy of your Key to Stress Profile.
- Comfortable, Layered Clothing, and a good pair of tennis shoes
Weather is sunny and warm during most days, cool during evenings and mornings.
Also, please be sure to complete the on-line Stress Management Inventory two weeks prior to your appointment.
