What to Bring

Please bring the following to the Program Session:

  1. Your completed New Patient Questionnaire
    You may download the New Patient Questionnaire Icon: Adobe Acrobat in Adobe Acrobat format. You may print a copy to complete offline, or you may complete the form within Adobe Acrobat. Please note that if you use Adobe Acrobat you will not be able to save your work.
  2. Any prescription medications, vitamins and supplements that you are currently taking.
  3. Copies of any prior lab work and medical history relevant to your health and hormonal status.
  4. A laptop computer.
  5. A printed copy of your Key to Stress Profile.
  6. Comfortable, Layered Clothing, and a good pair of tennis shoes
    Weather is sunny and warm during most days, cool during evenings and mornings.

Also, please be sure to complete the on-line Stress Management Inventory two weeks prior to your appointment.
