  Please order the $500 program deposit. It is suggested that you also order The Younger You® Two-Month MultiPack at the same time. Please provide the date you wish to attend in the "Comments / More Information" sectionof the checkout process.

3. Complete the New Patient Questionnaire
   Please download and complete the New Patient Questionnaire , and bring it with you on your session date.
4. Submit an online stress profile inventory
   We will E-Mail you instructions detailing how to submit an online stress profile after processing your registration. This includes access information and a personal access code.
