What to Bring to Your New Patient Appointment

Please bring the following:

1. Your completed New Patient Questionnaire
   You may download the New Patient Questionnaire in Adobe Acrobat format. You may print a copy to complete offline, or you may complete the form within Adobe Acrobat. Please note that if you use Adobe Acrobat you will not be able to save your work.
2. The Younger You® Two-Month MultiPack
   You may purchase The Younger You® Two-Month MultiPack for $279.95 from our online product store or by calling 1-800-392-2623. This product package is required for all new patients as it contains components that are essential to any successful anti-aging and regeneration medicine program, and optimizes the results that people will receive from BioIdentical Hormone Replacement Therapy.
3. Any prescription medications, vitamins and supplements that you are currently taking.
4. Copies of any prior lab work relevant to your hormonal status.
