New Patient Appointments
Fees and Health Insurance Information

Patients are expected to pay in full at the time of each visit and we accept payment by all major credit cards, check, or cash. We do not accept health insurance as a direct form of payment.

Patient Fees

New patient appointments cost $800, and include an initial visit with diagnostic evaluation and therapeutic treatment program. With each visit, patients receive a set of natural medical formulas and a prescription for bioidentical hormones, according to their treatment plan.

Follow-up visits cost approximately $200, and are typically scheduled four to six weeks after the first visit. With each visit, patients receive a set of natural medical formulas and a prescription for bioidentical hormones, according to their treatment plan.

Health Insurance Information

We do not accept health insurance as a direct form of payment. After your visit, you will be provided with a claim form (super bill) to submit to your insurance carrier for reimbursement. Your insurance provider will reimburse you directly whatever amount they deem is customary and reasonable. Dr. Galitzer is an M.D., so you should receive some reimbursement unless you have an HMO or high deductible to meet first. Insurance carriers vary widely in how much (or little) they reimburse.

Dr. Galitzer is no longer a Medicare provider, so any patient that has Medicare as their primary insurance will not receive any reimbursement, and cannot submit the claim form to Medicare. In these cases, secondary insurance providers may also not reimburse for patient claims.
